1. To submit online, you will be asked to provide information such as student athlete’s ID#, student athlete’s full given name, email address, date of birth, year in school, home phone, cell phone numbers, primary physician and hospital, emergency contacts with phone numbers and other pertinent information. Please make sure you have this with you when filling out your forms online or you will not be able to submit.
2. You CANNOT FILL OUT the PHYSICAL /MEDICAL HISTORY FORM online. YOU MUST PRINT THE FORM AND TAKE WITH YOU TO THE PHYSICIAN. It must be filled out by a physician and hand delivered to the Athletic Director at your school.
3. Remember, to submit online, you and your student athlete must fill out and sign in all required spaces. Forms will not be accepted electronically without everything filled out by the student athlete and you the parent/guardian.
4. To begin, click on the ELECTRONIC PARTICIPATION FORMS tab at the top of the page and fill in the information requested in the required fields. If a question is not applicable, please answer the question with “NA or UNKNOWN”.
5. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “electronic signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an a IPAD, you may create an electronic signature by holding your finger to the screen and signing or by using a stylus pen and signing.
6. Once you have filled out all of the information on each page you will have the opportunity to print the document.

10 To Enter Duncan Athletic Hall of Fame in September
Two Demons who reached the highest level in their sports and a nationally-recognized wrestling coach are among the 10 individuals to be inducted into the Duncan Athletic Hall of Fame.
The 2017 Hall of Fame class was chosen by a seven-person selection committee and will be honored beginning at 5:30 p.m. Saturday, Sept. 23, at a banquet and induction ceremony at Duncan Golf & Tennis Club.
Duncan High has had an Athletic Hall of Fame since 1978. The 2017 Class brings the total number of Hall of Famers to 91. This year's banquet, which features a meal catered by Interurban restaurant, is also linked to the 2017 Demons Football Homecoming Game, which is on Friday, Sept. 22.
Among the inductees are Greg Legg, a 1978 DHS graduate, who followed his high school and college careers by playing 13 years in the Philadelphia Phillies organization. Legg retired as a player in 1994 but has remained with the Phillies' minor league system, where he is spending this season as manager of the Reading (Pa.) Double A team.
Former Demon Mike Gibbons will be honored posthumously, with members of the Gibbons family planning to attend. After playing at Duncan and graduating in 1969, Mike Gibbons went on to play at what was then known as Southwestern Oklahoma State College. He then played with the Dallas Cowboys, New York Giants, Washington Redskins and Houson Oilers in the NFL, in addition to spending two seasons with the WFL Memphis Grizzlies.
Tom Brack is being inducted following a career as one of Oklahoma's finest high school wrestling coaches. After breaking into coaching at Duncan Junior High, Brack started the wrestling program at Marlow High and was also head coach at Comanche.
Brack returned to Duncan in 1995 as head junior high coach and became head varsity coach in 1997, retiring in 2009. His impact on the sport was acknowledged in 2016, when Brack was named to the Oklahoma Chapter of the National Wrestling Hall of Fame.
Other members of the 2017 Duncan Athletic Hall of Fame class are:
— Tim Buben, a 1988 DHS graduate, who was a football and track star for the Demons before playing football at Southwestern Oklahoma. He joined the coaching staff at DHS as a football and track assistant, and has been extremely successful in coaching young discus and shot put throwers.
— Darrel Martin, also a 1988 graduate. Martin was a football standout and a state champion in wrestling. He went on to wrestle at Arizona State.
— Carmen Link Huebert, a three-sport star for the Lady Demons, the 1994 DHS grad excelled in tennis, softball and basketball.
— Jon Fennel, 1989 Duncan grad, who was one of the foundation builders in the Demons highly-successful tennis program. The two-time state champion doubles player helped DHS win two team state titles.
— Jennifer Perkins Lehman, a 1992 graduate, was on two state champion doubles teams. She also finished third and fourth in the state in No. 1 singles. Helped the Lady Demons win team state titles in 1991 and 1992.
— Bud Burger, a Waurika native who spent nearly 20 years as the radio Voice of the Demons football team.
— Todd Ledford, a talented football and track athlete for the Demons, the 1991 grad played football at Northern Illinois and also had a stint as head football coach at Empire. He returned to DHS as a football and track assistant. Ledford Is currently head coach of Duncan's boys track program.
Advance tickets for the 2017 Duncan Athletic Hall of Fame Banquet will go on sale July 10.
Advance tickets for the 2017 Duncan Athletic Hall of Fame Banquet go on sale Monday, July 10, through the athletic office at Duncan High School.
The banquet is scheduled for 5:30 p.m. Saturday, Sept. 23, at Duncan Golf & Tennis Club. Advance tickets be purchased until the afternoon of Friday, Sept. 22 by contacting Machelle Zinn in the athletic department, 580-255-4366 Ext 264, or emailing firstname.lastname@example.org.
Advance tickets are $20 for those over 12 and $10 for those 12 and under. Tickets at the gate will also be priced at $20 for over 12 and $10 for 12 and under.
The Athletic Hall of Fame committee will be inducting 10 individuals, nine of whom were standouts as athletes or coaches, and one former radio announcer.
The group includes Carmen Link Huebert, Jon Fennel, Todd Ledford, Jennifer Perkins Lehman, TIm Buben, Greg Legg, Mike Gibbons, Darrel Martin and Tom Brack. Rounding out the induction class is Bud Burger, former Voice of Duncan Football.
Hall of Fame inductees receive two free banquet tickets. Inductees will also receive two general admission tickets to the Demons 2017 Homecoming Game, which is slated for Friday, Sept. 22.
The banquet begins with a social hour from 5:30 to 6:30, followed by a buffet meal provided by Interurban restaurant. The induction ceremony will begin at approximately 7:30 p.m.

Duncan Athletics will start selling a family pass to all Duncan School
